I don’t get it. Everytime I use the cloud item, when I enter a level Mario appears on his normal appearance. Does the cloud item do anything? Or is it just a waste of time.

Actually, Jugem’s Cloud does work on 2-Desert, 2-Pyramid, and Mini-Fortresses just fine. Piranha Plant levels are immune as you said, as are Wandering Hammer Bros. The only ‘best’ use of the thing would be to skip long, boring and/or problematic levels. Candidates I can think of for using the thing would be 3-8, 7-2nd Fortress, 8-1, and 8-Fortress.
